1. I understand that ESTC staff, and occasionally local media, take a variety of photographs and/or video of program activities. These photos/videos may be posted in the company’s online photo gallery, Facebook page, or used for promotional purposes (e.g. website, brochures, fair displays, etc), but NO names will be used unless specific permission is given to use names. I give permission to allow photographs/video to be taken of the participant (yourself or your child). I further give permission to ESTC the right to use any such photographs/video to illustrate and promote the program experience, ESTC and it's programs. While we will do our best to exclude the participant from any program activity photos, please be aware that other participants may bring their own personal cameras and may take photos of each other during the program.

2. I acknowledge that I am aware of the details of the program activities and that there is a possibility of personal risk of damage or injury in the activities and willingly agree to assume full responsibility for those risks as a condition of registering for the program. I, the undersigned, on behalf of myself, my heirs, legatees and assigns, hereby release and hold harmless Eastman Safety Training Centre, its officers, employees, agents, representatives, volunteers, heirs, executors, and assigns from all liability for myself (or my child) as well as all property damage or loss arising out of myself (or my child’s) participation in the program. I further undertake and agree to indemnify and hold harmless, Eastman Safety Training Centre, their landlord, employees and agents from all third party claims, costs, actions or causes of action which may arise at any time as a consequence of any acts or omissions of their instructors, assistants or any other person associated with ESTC. I understand that this release and indemnification releases liability for the conduct of ESTC and its officers, employees, agents, representatives, volunteers, heirs, executors, and assigns.

3. I acknowledge that I have read, understand and agree to all Eastman Safety Training Centre's policies, waivers and terms and conditions of registration and that I am signing and delivering the same freely and voluntarily and unconditionally. 4. I understand that ESTC reserves the right to refuse access to the program if the registration fees are not paid in full before the start of the program and that any fees already collected will not be refunded due to non-payment of balance and will be treated as a cancellation with less than 7 days notice.
